One Render Layer node and one multilayer EXR Image node, selected together in the Compositor. EXReplace moves the outgoing links from whichever of the two currently has them onto the other, matching each pass on the way.

Then EXReplace can't infer the direction on its own, so it treats the active node as the destination — click the node you want to receive the connections last, then run it. When only one node has outgoing links the direction is automatic.
