Gumroad has no way for add-ons to update themselves. Fetch takes you straight to your Gumroad library, and as soon as the new zip lands in your Downloads folder, Fetch installs it.

No. It only goes online when you press Check. When Blender starts, each repository's own setting in Blender decides whether it is checked.
