Does it bring back old features like Blender Internal?
No, it is a look-and-layout add-on. The shelf buttons run tools that still exist in Blender 5.
Does it change my shortcuts?
Only if you tick "2.7x Keymap (Right-Click Select)". It is off by default.
Will I lose my own theme?
 No. Before the first trip, Throwback saves every setting it is going to change to a backup file. "Back to the Present" (or switching the add-on off) puts your exact theme, layout, font and keymap back.
